APPLICATION FORMS

This contract form has to be filled up by the Employer and the apprentice in two copies. The employer must send the contracts to this Board for registration within one months time from the commencement of apprenticeship training.

The declaration on the 2nd page has to be filled up and signed by the student if the gap between date of passing and date of joining the training is less than one year.

If the gap is more than one year the Court Affidavit has to be furnished on stamp paper in the above format duly attested by Magistrate/Notary.

Please note that all the columns of the contract form are filled up and as enclosures Final year mark sheet of only Graduates/Diploma holders/Tech.(Voc.) apprentices Court affidavit if required. If this contract is filled up for sandwich courses a bonafide certificate issued by the Principal must be enclosed with the contract.
